Why after 6 months are started getting some acne on my shoulders?

Many things contribute to the risk of developing acne while on TRT. TRT is known to increase the risk of acne due to increased sebum production. Sebum production is primarily driven by elevations in DHT (a derivative of testosterone). Estradiol also contributes to acne production, to a lesser degree.

While you may need to adjust your TRT dosage or regimen to address these hormones, often times acne will develop due to non-hormonal lifestyle causes. These include diet, UV exposure, hygiene, and other factors.

These factors that you have been on TRT for 6 months before developing acne is mostly suggestive of a non-hormonal cause. Typically, infection with Propionibacterium acnes, a bacterium that commonly overgrows with higher sebum production, can occur around 4-6 months into TRT.

You should speak with your doctor about checking your DHT and estradiol levels along with other treatment options, which may include antibiotics, for your acne.
